  1. Pre-assessment counselling

This is provided over the phone by one of our experienced and specially rTMS trained consultants about any questions you may have about the treatment and effectiveness, method of delivery, monitoring and follow up processes. We are able to send the information in the post to you, if you wish.

  1. The Assessment

Following pre-assessment counselling if you and the consultant agree that rTMS would be a sound treatment option then we will offer you an hour psychiatric/ physical health assessment appointment.

The assessment is a comprehensive and detailed one with our senior doctor looking at your issues as a whole and providing advice to you about rTMS as well as any other treatment options including medication, talking therapies etc. that maybe appropriate. They will also be able to answer any further questions you may have about rTMS. We will show you the rTMS equipment if you let us know in advance, after the assessment.

The doctor may request your GP to carry out investigations such as blood tests, ECG, brain scans. However, this is not necessary in all patients as rTMS is a non-invasive treatment that does not interfere with the rest of the body function.

A letter of the assessment will be sent to your GP if you are happy for us to do so.

  1. rTMS Treatment

You will be in a chair similar to a dental treatment one for the treatment. The professional administering the treatment will place an electromagnetic coil on the region of brain that requires treatment. The coil passes magnetic pulses to this part of your brain to stimulate brain cells.

Each Session of rTMS lasts about half an hour and you will need daily sessions, 5 days a week for 3-6 weeks. The number of weeks may vary as per the mental health problem and associated issues e.g. duration, intensity or physical health sequelae and your response to the treatment. During the sessions you can sit or lie down, read, talk or listen to music or just relax.

  1. Monitoring During Treatment

Our rTMS doctor will review you every week to monitor progress and improvement in your condition. This entails speaking to the doctor for up to half an hour and completing validated mental health questionnaires to give us an exact idea of whether the treatment is working.

  1. Follow up After Completing rTMS

Unlike some other clinics we continue to provide care for you after the treatment ends and you recover, if required and appropriate. Many of our patients have continued to see the doctor they met for rTMS for review of their medication and overall care or benefited further from psychological (talking) therapies.

What is repetitive Transcranial magnetic stimulation?

Repetitive Transcranial magnetic stimulation (rTMS) is a painless and well-tolerated way of stimulating the brain for treatment of depression and anxiety. It has been in use since 1985. The therapy involves using a magnet to target and stimulate certain areas of the brain.

rTMS can be used for people with depression for whom other treatments have not worked or who cannot or would not like to take antidepressants for any reason.

What are the possible side effects and complications of rTMS?

Some people may describe the sensation of the magnetic pulse as uncomfortable with a knocking or tapping feeling or noise near the head during treatment.

Tingling in the face, jaw, or scalp

Though rare, rTMS has a very small risk of seizures.

 

 

Who should avoid rTMS?

The magnetic coil used in the treatment can be dangerous for anyone who has metal implanted somewhere in their head or neck. Therefore people who have the following cannot take the treatment even if it may be beneficial for them in the treatment of depression:

  • Aneurysm clips or coils
  • Cardiac pacemakers or implantable cardioverter defibrillators (ICD)
  • Facial tattoos that have magnetic ink or ink that is sensitive to magnets
  • Implanted stimulators
  • Metal implants in the ears or eyes
  • Stents in the neck or brain
  • Bullet fragments or shrapnel near the head

Also people with the following conditions:

  • Other neurological disorders
  • Epilepsy
  • Age below 18 years

A doctor would do a full psychiatric assessment and take a medical history of past medical problems before using the therapy.
